Enrollment Requirements

   

Each migrant student must be enrolled in the PASS Program before he/she can receive any course credit.  If a test is received for a student who is not enrolled, then the test cannot be processed.  A request will be sent to the local PASS contact person requesting an enrollment form.

It is equally important that the enrollment form be filled out correctly.  The following data is required to enroll a migrant student in the PASS Program:

  • Student name
  • School name
  • Migrant ID number
  • Migrant region number
  • Date of birth
  • Grade
  • Gender
  • Ethnicity

The migrant ID number is of special significance because it validates that a student qualifies for migrant services.  Please do not use any other ID number.  In addition, if the local PASS contact does not know or does not have the migrant ID number, then he should not write "new" or "pending" in the ID field.  Instead, the local contact should call the regional migrant office to obtain it.  Note that incorrectly filled out Enrollment forms will need to be returned to the local contact.

Recommendation

Many programs often have enrollment forms filled out by different teachers, counselors, student aids, etc.  Many of these individuals are not properly trained on how to correctly fill out the enrollment form.  Therefore, it is highly recommended that the school's PASS contact person review all filled out enrollment forms for accuracy and completeness before submitting them to our office.
